    The code is:

    // we already checked that this value exists so it's probably safe to ignore lookup failure...
    ValueMetaInterface insertValue = getInputRowMeta().searchValueMeta( meta.getUpdateStream()[i] ).clone();

    so it probably wasn't safe....
    but how could I have arrived at that situation. It's the first record in the batch

    You filled in the wrong name somewhere in the insert/update dialog.

    Found it :-)

    Actually caused by a duplcate field in the update fields list.

    still, it's not the friendliest of help messages.
